Referrals
Our team is committed to coordinated care and timely communication with referring providers. We will contact your patient directly to schedule their appointment, provide a timely consultation summary, and keep you informed through ongoing communication.
Consider referral for patients experiencing or presenting with:
- Chest pain or suspected cardiac symptoms
- Shortness of breath with exertion
- Known or suspected vascular disease
- Leg pain with walking (possible claudication)
- Abnormal cardiac testing or imaging results
- Cardiovascular risk evaluation
With your referral, please include:
- Patient demographics
- Progress notes
- Insurance information
