For my current patients that choose to continue their mental health care with me and new patients, you will need to have some ability to navigate the internet or have someone who can initially help you do so. I will communicate with you through Zoom. You can find links and instructions at the bottom of this page. I will need your email address so that I can send you information related to your diagnosis of mental health problems, prescribed medicine, counseling, and the provision of Christian spiritual guidance, if desired.

New Patients
Thank you for choosing Selma Medical Associates, Inc. to provide you with your mental health needs. Please complete the forms to the right and return them to us either by email to medicalrecords@selmamed.com, or at any reception desk at least 3 days in advance of your appointment. This will allow important information to be entered into your medical records before your first telemedicine appointment. If you know in advance that you cannot keep your telemedicine appointment we would appreciate your call to cancel as soon as you know you will not be able to come. If the appointment is not cancelled with at least 24 hours notice, you may be billed a fee of $50.00. If you have an insurance we participate with we will file your insurance claims for you. Please send or bring your insurance cards with your completed forms. |

Zoom
The easiest way to use Zoom is on your computer, but you can use your phone if you already have it installed there. To use Zoom on your computer, either download the Zoom program from zoom.us, or install the Google Chrome browser. I will need your e-mail address to connect with you on your computer. If you do not have access to Zoom, I cannot provide you with mental healthcare.
