Did You Know?
+ In order for your spouse or guardian to receive information regarding your protected health information, you must sign an authorization for use and disclosure.
+ If a patient is 18 or older, an authorization for use and disclosure must be signed in order for a parent or guardian to receive any protected health information.
+ If a patient needs a copy of their medical records for personal use, lawyers, insurance companies and some other agencies, there is a per page charge.
+ If someone is a Power of Attorney for a patient, there must be a "medical clause" stated in order to receive protected health information.
